The effect of bone marrow-derived mesenchymal stem cell co-transplantation with hematopoietic stem cells on liver fibrosis alleviation and survival in patients with class III β-thalassemia major

Abstract Background Hepatic fibrosis is a common complication in transfusion-dependent thalassemia patients. Data on the co-transplantation of mesenchymal stem cells (MSCs) with hematopoietic (HSCs) beta-thalassemia major patients are scarce. Therefore, we aimed to evaluate effect bone marrow-derived MSC HSCs liver alleviation and transplant outcomes class III major. Methods Between April 1998 January 2017, total 224 consecutive underwent allogeneic HSCT Research Institute for Oncology, Hematology Cell Therapy, Tehran University Medical Sciences, Tehran, Iran. To assess fibrotic changes after transplantation, 47 participated plus HSC group 30 only at end follow-up period. All laboratory tests, especially serum ferritin function testing, hepatic T2* MRI, biopsy, FibroScan before 2 years transplantation. Kaplan-Meier curves were derived determine survival compared using log-rank test. Repeated-measure, mixed-effect linear regression models used examine over time. Results The 10-year OS rate was 71.84% 61.89% non-mesenchymal ( P value = 0.294), while TFS 63.64% 52.78% 0.285). No significant difference observed NRM, rejection rate, ANC engraftment, platelet acute GvHD, chronic GvHD between two groups. In addition, results repeated-measure, showed that none variables determining had receiving MSCs who did not receive MSCs. Conclusions Based this study, single infusion time could significantly improve transplantation outcomes, including OS, TFS, TRM, GvHD.
